B'nai Mitvzah Experience: Date Request Form

Choosing your Date

Dear parents,

Welcome to the B'nai Mitzvah Experience at Temple Beth Am.
We are thrilled to partner with you and your child to make sure that they are prepared for this special day!
Please take a few minutes to fill out this form so we can assign your child their B'nai Mitzvah Date.
Dates are first come first serve. Please fill this form out by November 13, 2023 to get your first choice of dates. Families that do not complete this survey by then risk receiving a randomly selected date.
In order to get one of your first choice in dates, you need answer every question.
