Welcome to the B'nai Mitzvah Experience at Temple Beth Am. 
We are thrilled to partner with you and your child to make sure that they are prepared for this special day! 
Please take a few minutes to fill out this form so we can properly pair you with a mentor. 
This form should be filled out 12-14 months before your child's B'nai Mitzvah Date.

If this form is not filled out 12 months in advance, your child will be randomly assigned.
